- If the premium cost is less than the employer contribution (amounts shown in parenthesis below), the City contributes the difference to you and can be used toward supplemental insurance premiums or received as a pre-tax contribution to your Health Savings Account (HSA), if eligible. The City contributes any remaining dollars not used toward the cost of insurance to you in cash as taxable wages.
The City provides three high deductible health plans to choose from. A $2,000, $4,000 and $6,350 deductible.
There are two network options available
- Essentia Choice Care includes in-network Essentia providers. Care must begin at Essentia.
- Medica Choice Passport network includes a broad range of in-network providers including Essentia, Sanford, Mayo, and several independent providers.
Essentia Network
- 100% of the premium is paid by the City for the Essentia Choice Care Health Insurance for single coverage. No cost to the employee for the single plus one and family coverage for the $4,000 and $6,350 deductible plans!
- The $2,000 deductible plan has a cost to the employee for single plus one and family
Passport Network
- 100% of the premium is paid by the City for the Passport network of physicians for single coverage.
- There is a cost to the employee for single plus one and family coverage.
